Solutions necessary for providing electronic services – within our Online Services, we use the localStorage solution. localStorage is a file created by the user’s browser installed on their device. Thanks to the localStorage mechanism, we are able to provide our users with access to efficiently operating and secure Online Services and the ability to use their functionalities (e.g., remembering your privacy settings and the ability to change them, remembering preferences related to the selected display language of the site, the ability to add products to the cart). Information recorded and used within the localStorage mechanism will not be used for marketing activities. - Matomo Analytics: Within our Online Services, we use Matomo Analytics, a tool for collecting and analyzing information on how our website is used. Matomo Analytics helps us understand how users access and use our site, enabling us to continuously improve the site and its functionalities. This tool collects data such as: visited pages, time spent on each page, device type, operating system, geographical location, traffic source, and more. - USP Zdrowie and Facebook Ireland Limited, located at 4 Grand Canal Square, Grand Canal Harbour, Dublin 2, Ireland, act as joint controllers (entities jointly determining the purposes and means of personal data processing).
- USP Zdrowie and Facebook Ireland Limited have made joint arrangements regarding the fulfillment of their obligations under the GDPR.
- USP Zdrowie and Facebook Ireland Limited are responsible for providing appropriate grounds for processing personal data related to the collection of information via Facebook Pixel. - USP Zdrowie is responsible for the proper implementation and configuration of the Facebook Pixel tool. Facebook Ireland Limited remains responsible for ensuring the security of the service offered to us and the personal data processed within Facebook Pixel.

Providing us with your contact details and information related to medical inquiries is voluntary but necessary for our experts to offer you a professional and useful response.
In the case of contact forms with experts at USP Zdrowie, we will process your personal data solely to respond to the questions asked. The legal basis for processing your personal data is — with regard to information not related to your health condition — the necessity to provide the consultation service (Article 6(1)(b) GDPR) or your explicit consent to process information that is of a medical data nature, revealing or potentially revealing information about your health condition (Article 9(2)(a) GDPR). You may give your consent during a phone conversation by disclosing specific health information related to your health condition. We stipulate that we will not use the acquired information for purposes other than handling your inquiry and potential defense against claims.
Reporting adverse reactions via contact form or by telephone
Forms available on some of the Websites for reporting adverse events of medicinal products enable adverse event reporting by medical professionals as well as patients, their legal guardians, and statutory representatives. We also offer you the option to report an adverse event by calling the phone numbers provided on the Website or by contacting the Office for Registration of Medicinal Products, Medical Devices, and Biocidal Products.
The administrator of personal data contained in the form or provided by telephone, including in particular patient identification data (initials, age/date of birth, gender), identification data of the reporting person (first name, last name, address, phone number, email address, specialization), description of the adverse reaction, is the responsible entity, i.e., US Pharmacia sp. z o.o. USP Zdrowie participates in the process of submitting reports via forms as a data processor on behalf of US Pharmacia sp. z o.o. in the scope of technical and organizational support for the process of reporting adverse reactions.
US Pharmacia processes your personal data for purposes related to fulfilling the obligation to monitor the safety of medicinal products (Art. 6(1)(c) GDPR). In the case of special categories of personal data, such as health data, US Pharmacia processes your data based on the necessity due to reasons of public interest in the field of public health, such as ensuring high standards of quality and safety of medicinal products (Art. 9(2)(i) GDPR). The legal basis for processing the personal data mentioned above is the Act of September 6, 2001, Pharmaceutical Law.
Providing personal data by individuals obliged to report adverse effects of medicinal products is mandatory and results from the relevant provisions of the Act of September 6, 2001, Pharmaceutical Law and other legal regulations.
In the case of other individuals, providing personal data is voluntary but necessary for the registration of the report. You can also report an adverse reaction directly to the Office for Registration of Medicinal Products, Medical Devices, and Biocidal Products.
Enforcing compliance with the Rules, including combating fraud and abuse by USP Zdrowie
In connection with the concern for the interests of our users, personal data collected as part of the Online Services is also used to monitor compliance of individual activities with the provisions of their regulations and to combat fraud. The legal basis for processing this type of personal data is the legitimate interest of the administrator (art. 6 sec. 1 lit. f GDPR), which involves combating undesirable phenomena and preventing fraud.
Determination and pursuit of claims by USP Zdrowie

Period of personal data storage
Your personal data is stored for the period necessary to achieve the processing purposes (e.g., for the duration of account maintenance, the period necessary to handle inquiries and complaints) and as required by applicable regulations (e.g., the period necessary to handle and store reports of adverse effects of medicinal products).
When assessing the time necessary to achieve our goals, we take into account the nature of the data (including the nature of specific categories of data), their scope, and the context of processing. We also consider the provisions of applicable laws that obligate us to continue processing personal data. We also take into account the potential risk of unauthorized use or unauthorized disclosure of your personal data.
Personal data related to cookie technology is stored for a duration corresponding to the cookie lifecycle of 14 months, withdrawal of user consent, or until their deletion by the user.

- obtaining a copy of your personal data.
Right to rectification of data (Art. 16 GDPR)
You have the right to request the deletion of all or some personal data. In the event that the services we provide include the possibility of creating an account, a request to delete all personal data will be treated by us as a request to delete the account.
The right to request the deletion of personal data applies if:
- data are no longer necessary for the purposes for which they were collected or otherwise processed;
- an objection to the processing of personal data has been raised;
- the consent on which the processing is based has been withdrawn, and there is no other legal basis for processing;

Right to data portability (Art. 20 GDPR)
You have the right to receive the personal data you have provided to us and to transfer it to another data controller of your choice. The right to data portability applies when we process personal data based on consent or the premise of contract execution, and processing takes place in an automated manner, i.e., in IT systems. By exercising the right of access, we will provide you with a copy of the data in a machine-readable file format (e.g., CSV, Excel).
You may also request that personal data be sent by us directly to another administrator. We reserve the right to fulfill such a request only when it is technically possible.
